The following form is the entry point for registering your organization as a Community Assets NY Service Provider. Service Providers can request access to submit information into the system's applications by completing this form. For example, Service Providers can add events into the Community Calendar or add volunteering opportunities into the Volunteer Center at no cost. Once this registration form has been approved, you will automatically be listed in the system's Yellow Pages.
